Data Issue Management Form

Use this form if you have data issues or questions about institutional data, such as:

  • Input/decision when:
    • Using institutional data in a new way
    • Changing a data collection, storage, or other process
    • Building a new system that uses institutional data
  • Asking who is the right data steward for data you work with;
  • Seeking guidance on data policies, procedures, standards, business rules, architecture, and stakeholder interests;
  • Issues of compliance and conformance with laws, regulations, policies, standards, procedures, contracts, and architecture; and
  • Data quality issues (ie. data that appears wrong, inaccurate, stale, or duplicative)

Data issues will be sent to data stewards based on the data domain impacted and/or escalated according to the university’s Institutional Data Issue Management Procedure.
